How pregnancy dating works
Obstetric gestational age conventionally begins on the first day of the last menstrual period, about two weeks before conception in a regular 28-day cycle. That makes the standard LMP interval 280 days, or 40 weeks.
Due date formulas
A known conception date uses 266 days. ACOG gives specific assisted-reproduction offsets: add 263 days to a day-3 embryo transfer or 261 days to a day-5 transfer.

Ultrasound and the documented due date

Why LMP dating can differ
The 280-day rule assumes a 28-day cycle with ovulation on day 14. Recall error, cycle length variation, irregular ovulation and recent hormonal contraception can shift the relationship between LMP and conception.
